The information we seek from you is obtained so that we can provide a high quality of veterinary service and ensure that you are fully aware of our actions. We collect information for the following purposes: 1. keep records & meet veterinary legal requirements; 2. provide goods/services including followup care, scheduling and billing; 3. communicate/work with third parties involved in providing the above care. We want you to know that: 1. Our facility has a policy that is in accordance with the requirements of the PIPED Act. 2. Your information will not be shared for purposes other than above UNLESS YOU AGREE. The only exception is where the law requires it (eg. local Health Unit regarding a bite incident). 3. You can review your information at any time and change/correct if needed. 4. A copy of our policy will be provided upon request.
